About Your Visit
New Patients
Please arrive 30 minutes prior to your scheduled appointment to complete the paperwork. If you have already completed the paperwork at home, still arrive 15-20 minutes early as insurance information must be entered into the system prior to your visit. The medications that you take are extremely important to get entered into the system accurately. If you are uncertain about any of your medications, bring your bottles.
Established Patients
Please arrive 15 minutes prior to your scheduled appointment to update demographic information and health history including medication changes. We need you to review your medication list carefully for any changes at every office visit.
Patient Forms
Save time in the office and complete forms before your appointment. You may bring the forms with you to your appointment.

Insurance
Internal Medicine Practices accepts most insurance plans. We also accept "private pay" patients.
If you have questions about insurance coverage, please contact our office. The managed care health insurance market changes rapidly, and specific coverage questions should be discussed directly with your insurance company or our knowledgeable staff.
